Youth Forms
The following forms need to be completed and submitted prior to participating in youth activities at Pilgrim. These are fillable forms that can be fully completed by downloading and opening in a pdf program (such as Adobe). Once completed, save and email to Pastor Mike (plcpastor@pilgrimchurchofmarysville.org), or print and bring in to the church.
If you are having issues signing the document digitally, you can simply print and sign.
